In this particular example, the distracting elements are removed such as the branch in the left foreground, the houses in the background, patches of light coming from the middle, the drool on my son's pants, the tear in his eye from crying, the dirt on his knees from falling and the drool around his lips. The faces also have a dodging and burning done to it.
Retouching can range from removing any dirt from clothing, dry skin, red blemishes and can even go as far as face swapping with another similar image. If face-swapping does occur, I will always ask for permission before I do it.

In this example, the hair was caught on the tree and the window of the house is removed in the top left corner. Dodge and burn was also done to this image.
